BOARDING AGREEMENT
To insure that your pet is well taken care of when he or she stays with us while you’re away, we ask our boarding clients several questions when they check-in. For your convenience and a speedier check-in, you can submit your boarding paperwork ahead of time via the submission form below. This is especially handy for our boarding clients who have a plane to catch! You may check your pet in up to 30 minutes before closing and pick your pet up anytime during regular business hours:
Monday - Friday: 8:00 am to 8:00 pm
Saturday: 8:00 am to 12:00 pm and 5:00 - 6:00 pm pick up
Sunday: 8:00 - 9:00 am and 5:00 - 6:00 pm pickup.
Please allow 10 minutes for routine check in and out.

Authorization
Exam, Fecal, and Vaccination Policy

To insure the protection of all pets under our care, patients must be in good health with proof of a current exam (within one year), negative fecal test results (within one year), negative heartworm test (within 1 year) and the following vaccinations must be up to date: Dogs - Bordetella, Distemper/Parvo, Rabies. Cats - Distemper/Respiratory, Rabies. By checking this box I consent that my pet meets this criteria and/or I give permission for Millwood Animal Hospital to perform an exam and update vaccines if needed.
Medical Illness Policy

Yes, please perform any services the veterinarian deems necessary for the best care of my pet until someone can be reached. This includes only non-elective treatments and any necessary diagnosis.

I understand that boarding pets are not supervised between the hours of 8:00 pm and 2:00 am. I fully intend to pick up my pet on the date specified and pay in full at time of pick up. If circumstances change I will notify the hospital. I understand that regular hospital hours and pick up and drop offs are 8-5 Monday through Friday and 8-12 on Saturdays and that the hospital will be closed on some major holidays which may affect pick-up and drop-off times. I consent to the release of photos of my pet for Pet Ranch’s use on their website and Facebook page. I understand that Pet Ranch is not responsible for damaged or misplaced personal items brought into the facility.
